Application Form
Please read the following steps carefully when completing the IELTS Application Form:
Step 1 - Complete the IELTS Application Form. Please write clearly and legibly. Complete all questions fully. (Please note that an incomplete Application Form will be returned.)
To assist with completing the Application Form, please refer to " Instructions for Completing the IELTS Application Form".
Step 2 - Photographs - Include two passport photographs with your Application. (See section on "Photograph Requirements" for more specific information.)
Step 3 - Identification - Evidence of identity is required. This must be a valid, original passport or Permanent Resident Card (PRC) bearing a photograph, a number and a signature. The evidence of identity must show the full name of the applicant in English lettering and include the date of birth
Photocopy and include the page(s) of your passport or PRC showing your photograph, passport or PRC number, birth date and signature. Please remember that you must bring this same document of identity on the day of the test. If you do not bring the document, you will not be allowed to sit for the test, or transfer the test date or get a refund.
If you have sent your passport to your agent/attorney or Immigration Office, you must retrieve it in order to sit for the test, and send it back to them after the test is finished. If you do not have your passport, do NOT apply for the IELTS test until you have your document with you for test day.
Please Note: Canadian Citizenship Card and Driver's license are not accepted.
Step 4 - Test Fee - acceptable forms of payment are by Credit Card (Visa or MasterCard), or by Money Order or Certified Cheque (payable to Conestoga College ). Personal cheques are not accepted.
If paying by Credit Card, please complete the "Payment Authorization Form " and attach it to your Application Form.

NOTE: Due to test security concerns, incomplete mailed Applications will be returned to you for completion and you may miss your chosen test date. Please ensure your Application is complete before mailing. We do not accept faxed Applications .
Application Deadline
The IELTS Application Form and the test fee should be received by the IELTS Test Centre at least '5 business days' prior to the test date of your choice. However, registration is on a first-come, first-serve basis and does not guarantee a first choice test date.
In the event that a particular testing (first choice) has reached maximum capacity, the candidate's Application will be put forward to the candidate's second choice. If no seats are available for the first and second dates that you have chosen, you will automatically be registered for the next test date. It is recommended that you submit your Application Form and test fee as soon as possible.
